Levocetirizine: Complete Allergy Medicine Guide

Levocetirizine is a third-generation antihistamine and the active R-enantiomer of cetirizine. As a potent and selective H1-receptor inverse agonist, it provides rapid and long-lasting relief from allergic symptoms with minimal sedation. It is one of the most commonly prescribed antihistamines for allergic rhinitis and chronic urticaria.

Mechanism of Action

Levocetirizine acts as an inverse agonist at the histamine H1 receptor. Rather than simply blocking histamine binding (neutral antagonism), inverse agonism stabilizes the receptor in its inactive conformation, reducing constitutive receptor activity. This results in greater efficacy than simple H1 blockade. Levocetirizine also inhibits eosinophil chemotaxis and reduces ICAM-1 expression, contributing to its anti-allergic and anti-inflammatory effects beyond simple histamine blockade.

Clinical Uses

  • Seasonal allergic rhinitis: Sneezing, runny nose, itchy eyes
  • Perennial allergic rhinitis: Year-round nasal allergy symptoms
  • Chronic idiopathic urticaria: Hives of unknown cause
  • Allergic conjunctivitis: Itchy, watery eyes
  • Atopic dermatitis: Pruritus (itch) control
  • Insect bite reactions: Itch and swelling relief
