Omeprazole is a proton pump inhibitor (PPI) and one of the most widely prescribed medications globally for acid-related gastrointestinal disorders. Available as delayed-release capsules, omeprazole effectively reduces gastric acid secretion by irreversibly inhibiting the hydrogen-potassium ATPase enzyme system (the proton pump) in gastric parietal cells.
Mechanism of Action
Omeprazole is a prodrug that is activated in the acidic environment of the parietal cell canaliculus. Once activated, it forms a covalent disulfide bond with cysteine residues on the H⁺/K⁺-ATPase enzyme, irreversibly inhibiting the final step of acid production. A single dose inhibits approximately 70% of proton pumps, with maximal acid suppression achieved after 3-5 days of repeated dosing as new pumps are synthesized and subsequently inhibited.
Clinical Uses
1. Gastroesophageal Reflux Disease (GERD)
Omeprazole is first-line therapy for GERD, providing superior acid suppression compared to H2-receptor antagonists. Standard dosing is 20mg once daily for 4-8 weeks for erosive esophagitis healing, with maintenance therapy at 10-20mg daily for relapse prevention.
2. Peptic Ulcer Disease
For both gastric and duodenal ulcers, omeprazole accelerates healing by maintaining intragastric pH above 4 for extended periods. Healing rates exceed 90% at 8 weeks for gastric ulcers and 4 weeks for duodenal ulcers.
3. H. pylori Eradication
Omeprazole is a key component of triple therapy for Helicobacter pylori eradication: omeprazole 20mg + amoxicillin 1g + clarithromycin 500mg, all twice daily for 14 days. The PPI component enhances antibiotic efficacy by raising gastric pH.
4. Zollinger-Ellison Syndrome
For this rare condition of gastric acid hypersecretion, omeprazole is used at higher doses (60-120mg daily), often divided into multiple doses.
5. NSAID-Induced Gastropathy Prevention
Omeprazole 20mg daily is effective for preventing gastric and duodenal ulcers in patients requiring chronic NSAID therapy.

Side Effects
