Levocetirizine is a third-generation antihistamine and the active R-enantiomer of cetirizine. Mechanism of Action
Levocetirizine acts as an inverse agonist at the histamine H1 receptor. Rather than simply blocking histamine binding (neutral antagonism), inverse agonism stabilizes the receptor in its inactive conformation, reducing constitutive receptor activity. This results in greater efficacy than simple H1 blockade. Levocetirizine also inhibits eosinophil chemotaxis and reduces ICAM-1 expression, contributing to its anti-allergic and anti-inflammatory effects beyond simple histamine blockade.
